Does

Helps a young person write their own words down and keeps them unparaphrased. It never speaks for them.

Explains the paediatric advice a plan needs — Section C and G — and drafts it for a clinician to check and sign.

A named clinician — Holds GMC registration and clinical accountability. The health commissioning body must secure the health provision in Section G (Children and Families Act 2014, s.42(3)).

Does

Drafts health advice for a named clinician to check and sign. It is not a doctor, holds no registration, and gives no clinical opinion.

Holds the whole picture in one place, so a child never has to re-tell their story.

The named keyworker — Accountable to their line manager for coordination. Not a statutory duty-bearer for the provision itself.

Does

Tracks what is owed and what has been evidenced, and drafts the chase. It is not the keyworker and holds no case.

Tracks the LA's statutory duties and the clock they run on — accountable, dated, reasoned.

Responsible

The local authority caseworker — THE STATUTORY DUTY-BEARER. Must secure the special educational provision in Section F (Children and Families Act 2014, s.42(2)) and issue the final plan within 20 weeks of request (SEND Regulations 2014, Reg 13(2)). A school, a therapist and an assistant hold none of this.

Does

Explains the LA's duties and the clock, and drafts what a caseworker must check. It issues nothing and decides nothing.

Helps families put their evidence, concerns and rights on the table — an equal partner, not a visitor.

Parent or carer — No statutory duty to deliver provision. They hold rights instead — to be consulted, to ask for a school, and to appeal to the SEND Tribunal.

Does

Explains the process to a parent and helps them prepare what to say. It is not a caseworker and speaks to nobody on their behalf.

Graduated response, provision mapping and progress data for Section F.

The school's SENCo — A qualified teacher in a named school, accountable for the graduated response (SEND Code of Practice 6.84-6.94).

Does

Drafts provision wording and checks it is quantified, for the SENCo to check and sign. It runs no graduated response.

Safeguarding, family context and social care provision — present only where it belongs.

The allocated social worker — A registered social worker (Social Work England). The local authority holds the s.17 duty and must secure the Section H provision.

Does

Drafts social care advice for a registered social worker to check and sign. It makes no threshold decision and holds no case.

Therapy assessment and quantified provision — education or health, the plan must say which.

A speech and language or occupational therapist — A registered therapist (HCPC), accountable for their own assessment. The provision itself is secured by the commissioner.
