Brian is an avatar. The plan around him is illustrative; the education he describes in Section A is his own.The plan has run ten years and is proposed for cease. Two sections of his own record still have to reach him, and that happens tomorrow.

Section A — Views, interests and aspirations

ehc-cyp-voicerecorded 2026-09-08

Brian, in his own words, recorded at his review on 2026-09-08 and written down as he said it: "My name is Brian. I'm twenty. I didn't take the usual school route. I homeschooled myself, sat the GED instead of the regular Thai exams, got into Payap University, and then into Chiang Mai University. I'm reading economics at CMU — how markets work, how value moves, who gets left out when the system is opaque. What pulled me further in is the layer under the apps: blockchain, and what people call the next generation of the web — the one built so agents can act, settle, and hand work to each other without a person disappearing at every handover. I want to understand that stack the way I understand a balance sheet: incentives, settlement, identity, and who holds the keys. Economics gave me the questions; this is where I think a lot of the answers are moving. What I want on the record is this: the path was mine. I chose how I learned and I chose what I'm studying — economics, and the blockchain web that agents will run on. I still care about clear plans and people who don't disappear at the handover. I want my own plan to talk to adult services, not a file someone closed because I had a birthday." NOT PARAPHRASED. Brian convened this review himself and gave this statement first.

The register a child reads in, available to anyone. A professional who switches to this and cannot explain their own section has learned the most useful thing this feature does.

Brian’s story in health

Brian lives independently and manages his own health and money.

Worked by Dr Okonkwo — GP · Nurse Adeyemi — Practice nurse at Dr Okonkwo's surgery · Adult orthopaedic team — Adult follow-up after the leg operation. Agents and humans work together to fulfil statutory processes: ehc-doctor drafts for them and decides nothing.

10 artefacts filed under Sections C and G. Each one opens in the locker.

C — health needs relating to SEN · G — health provision (reg 12(1))

Outcomes this provision serves

from Section E

An outcome is here because a Section G provision is named as delivering it. Outcomes served by another section appear on that tab.

E4

Brian lives independently and manages his own health and money.

Baseline:
At sixteen, no independent travel, no budget of his own, appointments booked by an adult.
Target:
Own tenancy, own budget, own appointments, all without support.
Delivered here by:
G1
Time to deliver:
achieved
Blocked by:
nothing — this one does not wait for the plan

Also served by care provision — this tab shows one part of it, not the whole outcome.

Section G — Health provision

DELIVERED, AND HANDED OVER. Physiotherapy ran as set by the hospital team, with a school-based exercise plan the class teacher followed and adapted PE until he was cleared. School nurse checks ran 6-weekly on recovery, pain and wellbeing. Eight sessions with the mental health support team ran at 45 minutes weekly in 2017. All of it has ended by completion, not by lapse. What continues is adult orthopaedic surveillance, which sits with adult services and not with this plan. The inpatient episode is evidenced by photographs his father kept: the ward, the traction frame, and a nurse at the bedside. They are not clinical records and are not offered as any; they are what a family has when a hospital has the rest.

The conversation about this

Sections C and G are held by ehc-doctor. The chat rail on this page is scoped to this tab, so a question asked here is a question about health.

The meeting room opens at the top of the agenda, not at Section C — its agenda rows carry no anchors yet, and its controls are not live.

Who is around Brian's plan
